For example, research demonstrates that in bipolar II disorder, while psychological interventions combined with pharmacotherapy yield much better results than pharmacotherapy alone, relapse rates remain very high. In this article, we show the successful treatment of Mrs. C., who was diagnosed with Bipolar II disorder and fell into the non‐responders. The treatment integrated a novel approach grounded on a cognitive‐behavioral theory with a systemic perspective. The psychotherapist, the psychiatrist, and a family therapist composed the teamwork and delivered the treatment in three phases. In the first phase, the psychotherapist conjointly with the psychiatrist aimed at reducing symptoms. In the second phase, the psychotherapist and the family therapist addressed the dysfunctional relationship patterns that negatively reinforced emotional dysregulation. Finally, in the third phase, the aim was to consolidate the achievements, changes, and good outcomes.Artículo La psicoterapia basada en evidencia en la clínica actual. Una actualización para casos complejos(Fundación Aiglé, 2023-06) Scherb, Elena Diana; Kerman, Bernardo; De la Cruz Gil, RicardoEl objetivo de este trabajo es presentar una actualización del artículo sobre la psicoterapia basada en evidencias publicado en este mismo medio en 2022 (Scherb y Kerman, 2022), particularmente para los casos complejos. Seproponen cuatro vías de desarrollo de los nuevos paradigmas de psicoterapia basada en evidencias, partiendo desdeuna perspectiva trans-diagnóstica y trans-teórica. La primera, el cambio de foco de las investigaciones de efectividadde los modelos o técnicas psicológicas en trastornos psicopatológicas, hacia intervenciones efectivas, y mediadoresdel cambio. La segunda, la creciente evidencia de la importancia de los factores vinculados a la relación terapéuticaen el proceso y resultado de los tratamientos. La tercera, el giro hacia la personalización de los tratamientos, y laimportancia de la conceptualización y formulación del caso y no tanto a la clasificación diagnóstica. La cuarta, lainclusión del trabajo en equipo como factor que interviene, particularmente en casos complejos. A partir de unanueva manera de entender la práctica en Psicología Clínica, se impone un cambio en la conceptualización de lasintervenciones psicológicas basadas en evidencias, hacia un abordaje más idiográfico y centrado en procesos. In an attempt to contain contagions, disease and deaths, the different countries responded with isolation and social distancing measures, while the effort to manufacture an effective vaccine that could eliminate this threat began frantically. This required from the population different degrees of adaptation, that were broadly distinct between and within the countries. Peoples´ responses were both resilient and demoralizing, depending on multiple factors. Clearly the economic, social, political, educational, cultural, geopolitical, financial, religious contexts have an influence in the way peoples react as a whole. Cuando el diagnóstico implica severidad y comorbilidad en la sintomatología psiquiátrica la indicación es tratamiento combinado, medicación y psicoterapia. Este estudio de revisión sistemática tiene como objetivo recolectar artículos científicos sobre las psicoterapias efectivas y/o tratamientos combinados con pacientes con severidad y comorbilidad desde la perspectiva de las prácticas basadas en evidencias. Para ello se aplicó lo formulado en la decla-ración PRISMA y el criterio PICOS.Artículo Systematic review on effective psychotherapies and/or combined treatments with patients with severe and comorbidity(2024-03-18) Scherb, Elena DianaIn the last decade, many authors have admitted that we are witnessing a change of paradigm in the way we understand evidence-based psychotherapy practice at its core. This new approach is far more idiographic, based on change processes, and a more dimensional understanding of psychopathology and human suffering. The need for changing paradigms becomes more apparent when facing patients with psychopathological symptoms that imply severity and comorbidity. The generic indication to treat these patients is combined treatment, medication, and psychotherapy. This systematic bibliographical review aims to collect scientific articles on effective psychotherapies and/or combined treatments with patients with severity and comorbidity to assess the state of the art. A secondary aim is to highlight the differences in the number of scientific publications in Latin American databases versus American and European ones. For this purpose, the author applied PRISMA declaration principles and the PICOS criterion. In the first search, the author selected 95 (N=95) articles, of which 90% of the analysis units came from databases in Europe and the United States. Then, the author conducted three other searches, which selected 53 articles (N= 53), excluding the findings of European and USA databases because they yielded results that were too extensive. Tables I, II, and III summarize the findings.Artículo Teamwork for complex clinical situations in psychotherapy : introduction to the issue(WIley, 2023) Montesano, Adrián; Scherb, Elena DianaThis in‐session issue is focused on psychotherapists involved in and performing teamwork practices. Specifically, five teamwork‐based psychotherapy interventions are illustrated as solutions for complex clinical situations drawing from multiple theoretical approaches (narrative, systemic, cognitive behavioral, and integrative) and applied in different health care provision settings, ranging from psychotherapy private office to a multidisciplinary oncological service. The contributions try to cover a diversity of presenting problems: separating couples, gang involvement, schizophrenia, cancer and suicidal ideation, bipolar disorder; and formats of delivery such as couple therapy supervision, family therapy, multidisciplinary team formulation and interprofessional health psychology. Three main shared coordinates underlie the diversity of interventions: Considering that psychotherapy is just a piece of a broader network of interactions and meanings generated around a given problem/solution and, thus, it is part of an ecology of ideas (ecological dimension), assuming interdependence and collaboration as the best strategies to interact with professionals and significant others involved with a given problem or solution (collaborative dimension), and fostering a strengths‐based case formulation (epistemological dimension). The issue aims at enriching practitioners' toolbox willing to incorporate team‐based interventions as part of their range of professional competences.
